Registration and authorization forms help the clinic keep your information accurate and protect confidential records. Ask the office which current form is required for your request.
Patient Registration
Be ready to provide your legal name, date of birth, contact details, emergency contact, relevant health history and insurance information. Tell the office if your address, phone number or coverage has changed.
Release of Information
If records need to be sent to another clinician or an authorized person, ask for the appropriate release or access form. Identify the records requested, the recipient and how you would like them delivered.
See medical records information for the steps to prepare a request.
School, Work and Other Forms
Some forms require a clinical examination or supporting information. Contact the office before bringing the form and ask whether an appointment is needed, what identification is required and how you will be notified when it is ready.
Allow time for review and do not assume a form can be signed on the day it is submitted.
Fee Schedules and Employment Documents
Request current fee information directly rather than relying on older printed schedules. For employment inquiries, use the careers page to prepare your questions.
