A records request should clearly identify who is requesting the information and what records are needed. Contact the clinic to obtain its current access or authorization form and confirm the submission process.
Prepare Your Request
- Patient name, date of birth and current contact details.
- The dates, visits or type of information requested.
- Whether the records are for you or should be sent to another provider.
- The recipient's name and verified delivery details.
- Your preferred format, if available.
- Identification or proof of representative authority when required.
Access and Transfers
HIPAA generally gives patients the right to inspect and obtain copies of medical and billing records held by covered providers, with limited exceptions. A request for your own records and an authorization to disclose information to another person can have different requirements.
Ask the records team to explain the process, any permitted fee and the expected timing. Do not rely on old turnaround promises.
Corrections and Privacy
If you believe a record contains an error, ask how to request an amendment. Send documents through the clinic's approved process so confidential information reaches the appropriate team.
