Preparing for Your Visit

Bring the information that helps your clinician understand what matters to you.

Before You Arrive

  • Confirm your appointment time, location and the type of visit.
  • Bring identification and current insurance information, if applicable.
  • Prepare a list of prescription medicines, nonprescription products and supplements.
  • Note allergies, past reactions and relevant medical history.
  • Bring outside test results or specialist reports that the clinic has requested.
  • Let the office know in advance about language or accessibility needs.

Write Down Your Main Concern

A short record can be more useful than trying to remember everything during the appointment. Note when the problem began, how it has changed, what seems to affect it and what you have already tried.

Put your most important questions first. If you have several concerns, let the team know when scheduling so they can discuss what can be addressed at the visit.

Before You Leave

Check that you understand the plan: what happens next, which tests or prescriptions are involved, how results will be shared and when you should return. Ask whom to contact if the plan is unclear or your symptoms change.
