Understanding Your Medical Records

Your records help connect visits, results and treatment decisions.

What Records Can Include

A medical record may contain visit notes, diagnoses, test results, treatment plans and medicine information. A new clinician may need only certain parts, so ask which dates or documents are relevant before requesting a transfer.

Requesting Access

Patients generally have a right under HIPAA to inspect and obtain copies of records held by covered providers, with limited exceptions. Ask the records office how to submit a request and explain the format and information you need.

Check the Details

Review names, dates and medicine information. If something appears incorrect, ask how to request an amendment. A clinical term in a record may need explanation, so discuss questions with the treating team.

Protect Your Copy

Store downloaded or printed records securely and verify a recipient before sharing them. For clinic-specific steps, see medical records.
